An illness, injury or accident almost always befalls you when least expected. As a consequence, you may find yourself seeing an unfamiliar doctor or an out-of-hours GP service. In such cases, it may be critical for your heath care provider to be able to have fast and easy access to your most relevant medical information.
Your GP needs your permission before he can make your medical information available. When you visit our surgery, you will be given a form through which you can give us your permission.
